SD Card Inital fail with Wave Shield

Hi,

I am working on the charlie bear project from Make and I am almost done, but the when I run the serial monitor I find that the SD card is not initializing and it does not give me an error code it just says SD I/O error. It at one point, after I added the to the card and re-formatted it to FAT 32 said that it could not open the file, but that it was ready. However it stopped that and started the initializing error again. I used the SD example sketch and it is still not initializing. Help!

Did you format the SD card as recommended in the Wave Shield tutorial?

What libraries are you using? Where did you download them from?
What SD sketch are you using?
What kind of Arduino are you using?
Can you post detailed pictures of your wave shield, so that we can check the soldering and assembly?
